Migrating Legacy Attachments

Legacy inline prompt attachments should be moved into reusable prompts. Create prompt objects, create prompt versions, publish reviewed versions, then attach them to skill versions with replacePrompts.

The CLI desired-state path can do this in one workflow by declaring top-level prompts[] and referencing them from skills[].promptComposition.
